ABSTRACT

Since the application of image-guided radiotherapy in clinical practice, cone-beam CT (CBCT) has become the most widely employed image-guided device. However, some problems remain to be cautioned in the application of CBCT, such as certain errors in the setup errors before and after the correction of CBCT, the failure of CBCT to correct dynamic changes, whether the influence of application frequency of CBCT on patients’ boundary of clinical target volume can be reduced, imaging conditions of CBCT lack of a certain degree of flexibility, and the effect of clinical value and application cost on the practicality of CBCT. Only by fully understanding the problems in CBCT application can CBCT be applied more reasonably and scientifically.

ABSTRACT

Cone-beam computed tomography (CBCT) image based on a single layer of image and the choice of regions of interest (ROI) is affected by individual subjectivity, making it difficult to compare the image quality. In this article, a quantitative evaluation method of CBCT image quality based on three-dimensional parameters was proposed, which was extended from single layer to multi-layer and from pixel to voxel. The selection range of ROI was discussed to reduce the error of artificial selection and realize the quantitative evaluation of image quality by three-dimensional parameters.

ABSTRACT

Objective To compare the results of three different registration methods in the kilovolt CBCT guided IMRT for nasopharyngeal cancer (NPC).Methods Total 560 CBCT images of 24 NPC patients who received kilovolt CBCT guided IMRT were analyzed off line.Three registration methods were used for alignment between CBCT and planning CT,including translational and rotational errors for bone and grey (BoneT + R,GreyT+ R),only translational errors for grey (GreyT).The registration results were analyzed by mean paired t-test respectively.Results With method BoneT+R,the translational errors on x,y and z axes were (-0.11 ± 1.35) mm,(0.40 ± 2.09) mm and (0.95 ± 1.56) mm and the rotational errors were 1.06° ±0.67°,0.01° ± 1.28° and 0.92° ± 1.00° respectively.With GreyT+R,the translational errors on x,y and z axes were (-0.02 ± 1.06) mm,(0.68 ± 1.92) mm and (0.81 ± 1.46) mm and the rotational errors were 0.85° ±0.61°,-0.05° ± 1.32° and 0.91° ±0.72° respectively.With GreyT,the translational errors on x,y and z axes were (0.58 ± 1.02),(0.52 ± 1.89) and (0.44 ± 1.43) mm.The results of compared mean t-test for different registration methods groups have significant difference (P =0.00-0.01) except for the rotational errors on y and z axes between BoneT+R and GreyT+R (P =0.05,0.62).Conclusions There have different alignment errors when different registration methods used for NPC kilovolt CBCT guided radiotherapy.If there have correct methods for rotation errors,GreyT+R registration method may be a better choice.In opposite,GreyT+R registration method would be used firstly to verify whether the rotational error > 2°or 3°.If the rotational error > 2°or 3°,the patient should be re-setup.If not,according to these alignment results,the GreyT method,manual method would be used to compensate the translational errors.

ABSTRACT

Objective The aim of this study is to evaluate the feasibility of using orthogonal kilo voltage fluoroscopic imaging(OKVFI)for setup correction in image guided radiotherapy of the liver.Methods After positioned the patients with liver cancer implanted with silver rings on the accelerator equipped with kilo voltage X-ray volume imaging(XVI),averaged OKVFI and cone beam CT(CBCT) volumetric images were acquired.A total of 90 datasets of averaged OKVFI and 90 datasets of volumetric images for 10 patients were obtained.The couch shifts obtained by the matching between OKVFI and digitally reconstructed radiograph were compared tu those achieved by the registration between CBCT and 4D reference average CT.On the comparison of the two different matching metheds.the Pearson coefficient was used to analyzed the correlation and Bland-Altman analysis to discern the consistence.Results The Pearson coefficient of correlation for the patient position shifts were R2=0.821.0.771 and 0.909 in the left-right (LR),anterior-posterior(AP)and superior-inferior(SI)directions respectively.95% CI were-2.30 -1.53(LR),-2.06-3.01(AP)and-2.69-1.53(SI)respectively.Within a±3 mm tolerance were 97.78%.95.56%and 96.67%respectively.Conclusions OKVFI has hish correlation and consistence with CBCT image on the setup correction.It is feasible to implement position correction with OKVFI in clinic practice.
